Here is a list of all the qualifying criterias for an Illinois Medical Card. 

Qualifying for an Illinois Medical Card

Patients must be 18 years of age or older.

Minors should have a designated caregiver or provide proof of self sufficiency or emancipation and recommendation from two physicians.

Patients must present a proof of residency in Illinois.

Patients must be stricken by a state approved medical condition.

Patient must not hold a commercial driver’s license or a school bus permit.

Register With the State

Use our doctor’s recommendation to register with the Illinois Department of Health. You can choose between a 1, 2 or 3 year application and pay the state registration fee of $100, $200 and $250, respectively.
